Wormtail's silver hand, Cedric, Frank Bryce, Bertha Jorkins, Lily Potter, and James Potter (at least in my version). Original versions had James coming out of the wand before Lily and this was incorrect because he died first.

Right, I think I've got them all (though probably in the wrong order):
1). When she was the beetle in Hermione's hair after the second task
2). Overhearing Hagrid & Olympe (Harry remembers the beetle later) at the Yule Ball
3). Having to have flown up to hear/see Harry's dream thingy in Divination
So I believe that it's three...although I have a niggling doubt that I'm forgetting one...
